A copy of the child's up-to-date Immunization Records.  Please Note:  In order to enroll in a GEORGIA school, all children must have an up to date immunization certificate (Form 3231).  This form should be obtained from your immunization provider PRIOR to arriving at the school's office for registration.  This requirement especially applies if your child has an out of state shot record.  And please be aware that immunization requirements vary depending on the age of the child.  For more information on this, you may contact the Union County Health Department in Blairsville at (706) 745-6292.

A copy of the child's Birth Certificate.  This will enable us to prove the child's exact date of birth.  Please Note:  All Kindergarten students must be at least five years of age on or before September 1st of the current school year to enroll at Union County Schools.  This rule may not apply to Kindergarten students who are transferring from another state.

A copy of the child's Social Security Card that plainly displays their assigned ID number.  This will be your child's identification number in our school system.  If you do not have a social security number for your child or do not wish to supply this information to the school system, please check with the registrar for a list of options.

Proof of the Parent/Guardian's physical address in Union County.  This can be any form which lists your 911 address (ie: electric bill, lease agreement, etc.)

A copy of the student's last report card and a withdrawal form from the child's previous school.  We will also need the previous school's address and phone numbers to enable us to request your child's academic records at the time of registration.  Please Note:  This request does not apply to the new Kindergarten registrations that are screened by appointment in the spring before every new school year.  For more information on Kindergarten registration, please refer to that section on the primary school home page.

If applicable ... parents and/or guardians will also need to provide the school with copies of legal documents that might affect your child while at school.  Without official papers on file, the school has no legal right to enforce restrictions.
